Abstract [sv]

Bakgrund: Kognitiva sjukdomar kännetecknas av nedsatt minnesförmåga, samt förändringar i beteende och funktion för att klara av vardagliga aktiviteter. Bland personer med kognitiv sjukdom finns hög förekomst av beteendemässiga och psykiska symtom, så kallade BPSD. BPSD kan yttra sig i form av depression, apati, agitation och vandringsbeteende vilket skapar och bidrar till omfattande utmaningar för den drabbade, anhöriga och vårdpersonal. Då farmakologisk behandling inte alltid räcker för att lindra symtom vid kognitiv sjukdom samt att icke-farmakologiska insatser rekomenderas i första hand vid BPSD, har intresset för alternativa omvårdnadsåtgärder ökat. Musikinterventioner har påvisat god potential att reducera BPSD och på så sätt öka välbefinnandet för den drabbade, närstående och vårdpersonal.

Syfte: Syftet var att undersöka hur musikinterventioner påverkar beteendemässiga och psykiska symtom hos personer med kognitiv sjukdom.

Metod: Som studedesign användes en icke systematisk litteraturöversikt. Resultatet baserades på 15 orginalartiklar med kvanitiativ metodansats. Artiklarna inhämtades från databaserna PubMed och CINAHL. Kvalitetsgranskning av artiklarna utfördes med Sophiahemmets granskningsmall och därefter anlyserades utefter en integrerad datanalys.

Resultat: Resultatet visar på att musikinterventioner, särskilt individanpassad musik, har potential att minska symtom som agitation och depression. Tre kategorier framträdde i resultatet: Musikinterventioners påverkan på agitation, Musikinterventioners påverkan på depression, Musikinterventioners påverkan på andra BPSD-symtom. Resultatet visade dock stor variation när det kommer till interventionens utformning och effekter.

Slutsats: Sammantaget visar studien att musikinterventioner, särskilt individanpassad musik, har potential att minska symtom som agitation och depression. Inom omvårdnad kan denna kunskap vara värdefull, då musikinterventioner utgör en icke-invasiv metod och är ett kostnadseffektivt alternativ för att lindra BPSD. Vidare framkommer ett tydligt behov av fortsatt forskning, framför allt kvalitativa studier vilket kan bidra till en ökad förståelse för hur personer med kognitiv sjukdom själva upplever att musiken påverkar deras liv.

Abstract [en]

Background: Cognitive disorders are characterised by impaired memory, as well as behavioral and functional changes to manage everyday activities. Within the category of cognitive disorders there is a high degree of behavioral and psychological symptoms of dementia (BPSD) which can manifest as depression, apathy, agitation and wandering behaviour. BPSD can cause severe challenges both for the patient, relatives and medical staff. The interests for alternative nursing interventions have increased since pharmacological treatment does not always alleviate symptoms in cognitive disorders sufficiently and because non-pharmacological interventions are recommended as the first line of treatment for BPSD. Music interventions have shown great potential to reduce BPSD, and thereby increased the well-being of the affected individuals, relatives and medical staff.

Aim: The aim of this study was to examine how music interventions affect behavioral and psychological symptoms in persons with cognitive disorders.

Method: A non-systematic literature review was conducted as a structural design for the study. The results were based on 15 original articles with quantitative methodology. The articles were retrieved from databases PubMed and CINAHL. Quality measures were applied using Sophiahemmet´s review template and the data were analysed by an integrated data analysis yapproach.

Results: The result showed that individually tailored music had the greatest effect in reducing agitation and depression. Three prominent categories were identified in the result: the effect of Music intervention on agitation, The effect of Music intervention on depression, the effect of Music intervention on other BPSD symptoms. However, the results varied considerably depending on the format of the intervention and the outcomes measured.

Conclusions: The study suggests that music interventions and specifically individually tailored music, have potential to reduce symptoms such as agitation and depression. In nursing care, this knowledge may be valuable, as music interventions represent a non-invasive method requiring limited financial resources, serving as an alternative to alleviate BPSD. Furthermore, there is a clear need for continued research, particularly qualitative studies, to better understand how individuals with cognitive disorders themselves experience the influence of music on their daily lives.
