Palliative care in prison : A literature review about prison staff´s experience of caring for inmates with palliative care needs in prison (English)
Abstract [sv]

Bakgrund: Palliativ vård fokuserar på att förebygga och att lindra lidande, genom tidig upptäckt, bedömning och behandling av smärta. Den palliativa vården i fängelse påverkas av flertalet faktorer, främst kring hur landets sjukvård är utformat. Internationellt sett är en åldrande fängelsepopulation med större omvårdnadsbehov ett faktum. Intagna löper en dubbelt så stor risk att utveckla palliativa vårdbehov jämfört med övrig befolkning. Flera professioner är verksamma inom fängelsevården, exempelvis läkare, sjuksköterska, säkerhetspersonal, administratörer, socialtjänst och frivilliga volontärer, från både externa hospice och andra intagna. Specialistsjuksköterskan inom palliativ vård har ett stort ansvar att leda och utveckla den palliativa vården.

Syfte: Beskriva fängelsepersonals upplevelse av att vårda intagna med palliativa vårdbehov.

Metod: En litteraturöversikt med strukturerad metod där elva artiklar med kvalitativ design inkluderades. Tematisk analys utifrån Braun och Clarke (2006) användes. Som teoretisk referensram användes Jean Watsons teori om mänsklig omsorgsrelation.

Resultat: Resultatet bygger på två huvudteman och sex underteman. Huvudteman är; ”Begränsningar och utmaningar i en säkerhetspräglad miljö" samt "Relationer och emotionellt arbete". Fängelsepersonalen upplever svårigheter att kombinera vård och säkerhet, saknar utbildning, handledning och arbetar i otillräckliga lokaler. Det andra temat uppger att fängelsepersonalen upplever att vårdrelationen påverkas negativt av fängelsekulturen, och personalen betonar vikten av kollegialt samarbete och stöd för framgång. Slutligen belyses motiverande faktorer för personalen.

Slutsats: Det framkommer att fängelsepersonal möter svårigheter i utförandet av palliativ vård, på grund av brist på resurser, utbildning och stöd. Den rådande fängelsekulturen påverkar vårdrelationen negativt, vilket minskar såväl personalens engagemang som deras empati. Samarbete inom fängelset och med externa vårdgivare är avgörande för att förbättra den palliativa vård som ges till den intagne. Sjuksköterskan spelar en viktig roll i den etiska reflektionen och har ett ansvar att främja mänskliga rättigheter och en jämlik god vård. 

Abstract [en]

Background: Palliative care focuses on preventing and alleviating suffering, through early detection, assessment and treatment. Palliative care in prison is affected by how the country's healthcare is designed. Globally, an aging prison population with increasing care needs is a fact. Inmates run twice the risk of developing the need of palliative care. Several professions work within the prison hospice, physicians, nurses, security personnel, administrators, social workers and volunteers. A specialist nurse within the palliative care setting has a responsibility to lead and develop the care provided.

Aim: Describe prison staff's experiences of providing care to inmates with palliative care needs. 

Method: A literature review with a structured approach in which eleven scientific articles with qualitative methods were included. Thematic analysis based on Braun and Clarke (2006) was used. Jean Watson's theory of human caring was used as a theoretical framework.

Results: The results are based on two themes and six subthemes. The first theme was; Limitations and challenges in a safety-oriented environment. The staff experience difficulties combining the caring and the security aspects and the staff lack education and guidance. They face negativity and experience that the premises are outdated. The second theme, Relations and emotional work, refers to that the prison staff experienced a negative impact on the care relationship due to the prison culture. Collaboration with colleagues is vital. Lastly, the prison staff's motivating factors are clarified. 

Conclusion: This literature review highlights that prison staff face difficulties in providing palliative care. The prison culture negatively affects the care relationship, which can reduce both the staff's commitment and their empathy. Collaboration within the prison and with external care providers is essential to improve palliative care. Nurses should create an opportunity for ethical reflection and obtain a responsibility to promote human rights and equal care.
