Abstract [sv]

Bakgrund: Definitionen av palliativ vård enligt World health organization [WHO] avser att förbättra livskvalitén både för patienten och dennes familj som står inför en livshotande sjukdom. Palliativ vård involverar flertalet olika professioner som har lika viktiga roller för att kunna erbjuda och finnas där för både patienten och dennes familj. Definitionen av hemlöshet är enligt Socialstyrelsen bred, med olika situationer som innefattar en kortare eller längre tid som hemlös. Dagens vårdsystem inte är anpassat för personer som lever i hemlöshet, då denna bygger på egen ansvar och självhjälp.

Syfte: Syftet var att beskriva sjukvårdspersonals erfarenheter av sjukvård av personer som lever i hemlöshet med palliativa vårdbehov.

Metod: En icke-systematisk litteraturöversikt användes med metoden mixed studies review och en induktiv ansats. Vidare användes en kvalitativ analys för att besvara syftet.

Resultat: Resultatet påvisade sjukvårdspersonals erfarenheter av barriärer för att erbjuda palliativ vård till personer som lever i hemlöshet. Barriärer i form av komplexa vårdbehov, att möta rädsla, misstro och bristande tillit samt organisatoriska och praktiska hinder. Den vanligt förekommande bristen av ett hem för de hemlösa i kombination med att vården alltmer förflyttas till hemmet utesluter denna målgrupp från olika former av vårdtjänster. För att kunna erbjuda god palliativ vård till personer i hemlöshet värderades vikten av att se personen bakom hemlöshet, organisatoriska förutsättningar samt behov av kunskap och utbildningar.

Slutsats: Resultatet visar på att personer i hemlöshet sällan, inte alls eller för sent erhåller palliativ vård. Boendeform påverkar hur den palliativa vården kan bedrivas i hemmet, vilket skapar problem för personer som lever i hemlöshet. För att kunna bedriva en god palliativ vård är multiprofessionella team viktigt. Resultatet beskriver sjukvårdspersonals osäkerhet och rädsla i mötet med personer i hemlöshet, där flertalet saknar kunskap och erfarenhet. Organisatoriska förutsättningar påverkar även den palliativa vården för personer i hemlöshet.  

Abstract [en]

Background: The definition of palliative care according to the World Health Organization aims to improve the quality of life for both the patient and their family who are facing a life-threatening illness. Palliative care involves several different professions that have equally important roles in being able to offer and be there for both the patient and their family. According to Socialstyrelsen, the definition of homelessness is broad, with different situations that include a shorter or longer period of being homeless. Today's healthcare system is not adapted for people living in homelessness, as it’s based on their own responsibility and self-help.

Aim: The aim was to investigate healthcare professionals´ experiences of healthcare of persons living in homelessness with palliative care needs.

Method: A not-systematic literature review was used with method mixed studies review with an inductive approach. A qualitative analysis was used to answer the purpose.

Results: The results showed healthcare professionals' experiences of barriers to offering palliative care to people living in homelessness. Barriers in the form of complex care needs, facing fear, mistrust and lack of trust, as well as organizational and practical obstacles. The common lack of a home for the homeless in combination with the fact that care is increasingly being moved to the home excludes this target group from various forms of care services. In order to be able to offer good palliative care to people in homelessness, the importance of seeing the person behind homelessness, organizational conditions and the need for knowledge and training was assessed.

Conclusion: The results show that homeless people rarely, not at all or too late receive palliative care. The type of accommodation affects how palliative care can be provided at home, which creates problems for people living in homelessness. In order to provide good palliative care, multi-professional teams are important. The results describe the uncertainty and fear of healthcare professionals in meeting homeless people, where the majority lack knowledge and experience. Organizational conditions also affect palliative care for homeless people.
