Bakgrund 

Antalet flyktingar ökar årligen på grund av krig, politisk instabilitet, förföljelse och brott mot mänskliga rättigheter. Både situationen i hemlandet, migrationen och asylprocessen påverkar deras ofta komplexa fysiska, psykiska och kulturella behov. Flyktingsituationen är en samhällsutmaning där hälso- och sjukvården har ett särskilt ansvar. Sjuksköterskan har en nyckelroll i att identifiera dessa behov, motverka diskriminering och främja rätten till hälsa. Trots rätten till vård möter både flyktingar och vårdpersonal betydande hinder. 

Syfte 

Syftet var att beskriva asylsökandes och flyktingars upplevelser av att söka och ta emot vård i ett mottagarland. 

Metod 

Denna icke-systematiska litteraturöversikt inkluderade 15 vetenskapliga artiklar med kvalitativ ansats. Utifrån syfte och urvalskriterier genomfördes datainsamlingen med sökord och sökblock i databaserna PubMed och CINAHL. Artiklarna sammanställdes och analyserades med en integrerad dataanalys, där återkommande fenomen och beskrivningar i de inkluderade studierna bidrog till en ny helhet med kategorier och underkategorier. 

Resultat 

Resultatet sammanställdes i två kategorier: Tillgången till hälso- och sjukvård och Kommunikation och bemötande. Kommunikationssvårigheter och bristande kunskap om det nya sjukvårdsystemet utgör betydande hinder, liksom upplevelser av diskriminering och bristande kulturell kompetens hos vårdpersonal. Tidigare erfarenheter och förväntningar på vården visade sig även vara av betydelse, samt geografiska och strukturella hinder. 

Slutsats 

Denna litteraturöversikt beskriver asylsökandes och flyktingars erfarenheter av att söka och ta emot vård, och lyfter fram flera utmaningar kopplade till både tillgång till vård och vårdmötet. Ett återkommande behov bland deltagarna är en mer respektfull, informativ samt person- och familjecentrerad vård. 

Background 

The number of refugees increases annually due to war, political instability, persecution and human rights violations. The situation in their home country, migration and the asylum process all affect their often complex physical, psychological and cultural needs. The refugee situation is a societal challenge for which healthcare has a particular responsibility. Nurses play a key role in identifying these needs, combating discrimination and promoting the right to health. Despite the right to healthcare, both refugees and healthcare professionals face significant obstacles. 

Aim 

The aim of this study was to describe asylum seekers' and refugees' experiences of seeking and receiving care in a recipient country. 

Method 

This non-systematic literature review included 15 scientific articles with a qualitative approach. Based on the aim of the literature review and selection criteria, the data collection was carried out using keyword and search blocks in the databases PubMed and CINAHL. The articles were compiled and analyzed using an integrated data analysis, where recurring phenomenon and descriptions in the included studies were contributed to a new entirety with categories and subcategories. 

Results 

The results were compiled into two categories: Access to healthcare and Communication and treatment. Communication difficulties and lack of knowledge about the new healthcare system constitute significant obstacles, as do experiences of discrimination and lack of cultural competence among healthcare professionals. Previous experiences and expectations of healthcare also proved to be important, as well as geographical and structural obstacles. 

Conclusions 

This literature review highlights the experiences of asylum seekers and refugees in seeking and receiving healthcare, and highlights several challenges linked to both access to healthcare and the healthcare encounter. A recurring need among the participants is for more respectful, informative, and person- and family-centered care.
