Living with pancreatic cancer : a non-systematic literature review (English)
Abstract [sv]

Bakgrund

Pankreascancer är en aggressiv diagnos som medför diffusa och otydliga symtom för patienterna, vilket resulterar i att cancern oftast diagnostiseras i det avancerade stadiet. Den kurativa behandlingen utförs på mindre än 20 procent av patienterna då många inte kvalificerar för operation. Det innebär att många patienter i stället får en palliativ vård i kombination med kemoterapi. En palliativ vård ges till patienter som har en obotlig sjukdom i syfte att förbättra patientens livskvalitet samt minska lidande, där sjuksköterskor behöver ha kunskap i hur de erbjuder en god omvårdnad och personcentrerad vård.

Syfte

Syftet var att beskriva patienters upplevelser av att leva med pankreascancer.

Metod

Den gjorda icke-systematiska litteraturöversikten innehåller tio vetenskapliga artiklar av både kvantitativ och kvalitativ ansats. De valda vetenskapliga artiklarna inhämtades från databaserna PubMed och CINAHL där sökningen gjordes i tre sökblock. Artiklarna har lästs igenom och valts utifrån exklusions- och inklusionkriterierna för att därefter kvalitetsgranskas med hjälp av Sophiahemmet Högskolas bedömningsunderlag. Resultatet har sammanställts utifrån en integrerad dataanalys.

Resultat

De två huvudkategorierna som resultatet skrivits utifrån är Transitionen från frisk till sjuk och Valet av behandling. Resultatet visade att patienter med pankreascancer upplevde en identitetsförändring i transitionen från frisk till sjuk. Förändringen medförde rädslor för framtiden, existentiell kris, stress och oro. Hanteringsstrategier som användes var olika beroende på patienten. Den strategi som tillämpades var avgörande för om patienten upplevde en acceptans alternativt förnekelse kring diagnosen. Livskvalitet, respekt för autonomin och uppnå en ”god död” var även viktiga faktorer för dessa patienter.

Slutsats

Litteraturöversikten identifierade att patienter med pankreascancer upplevde en ökad existentiell oro samt behov av stöd i sjukdomens utveckling. En ökad medvetenhet från sjuksköterskan kommer medföra en förståelse, vilket möjliggör en mer personcentrerad vård som resulterar i en god livskvalitet.

Background

Pancreatic cancer is an aggressive disease that often presents with diffuse and vague symptoms, leading to its diagnosis typically at an advanced stage. Fewer than 20 percent of patients qualify for surgery, which means many receive palliative care alongside chemotherapy. Palliative care is provided to patients with incurable diseases to enhance their quality of life and alleviate suffering. It is essential for nurses to possess the knowledge and skills necessary to deliver effective nursing and personcentered care in these situations.

Aim

The aim was to describe patients' experiences of living with pancreatic cancer.

Method

This non-systematic literature review includes ten scientific articles that employ both quantitative and qualitative methodologies. The selected articles were sourced from the PubMed and CINAHL databases through three distinct search blocks. Each article was read, evaluated based on predefined inclusion and exclusion criteria, and assessed for quality using the evaluation documents from Sophiahemmet University. The findings were compiled based on an integrated data analysis.

Results

The results were organized into two main categories: The transition from health to illness and The choice of treatment. Patients with pancreatic cancer experienced a significant change in their identity during this transition from healthy to sick, leading to fears about the future, existential crises, stress, and anxiety. The coping strategies employed varied among patients and significantly influenced whether they accepted or denied their diagnosis. Additionally, factors such as quality of life, respect for autonomy, and the pursuit of a "good death" were important considerations for these patients.

Conclusions

The literature review revealed that patients with pancreatic cancer often experience heightened existential anxiety and require additional support as the disease progresses. Greater awareness on the part of nurses can enhance understanding, which in turn allows for more personcentered care and ultimately leads to an improved quality of life for patients.
