Bakgrund

Sepsis är ett livshotande tillstånd med dysfunktionell påverkan på minst ett organsystem orsakat av en systemisk onormal reaktion på en infektion och har länge förknippats med hög dödlighet. De senaste åren har riktlinjer för tidig upptäckt och insättning av behandling förbättrats. Behovet av uppföljning efter sepsis har också tagit större plats i forskning såväl som i olika riktlinjer. Kunskapen om komplikationerna och deras karaktär uppges dock vara bristfällig bland sjuksköterskor och övrig hälso- och sjukvårdspersonal. Denna litteraturöversikt har använt Dorothea Orems egenvårdsteorier som teoretisk utgångspunkt. 

Syfte

Syftet var att belysa komplikationer hos vuxna personer som haft sepsis.

Metod

Studien genomfördes som en icke-systematisk litteraturöversikt och baserades på 14 vetenskapliga artiklar med både kvalitativ och kvantitativ ansats. Urvalet av artiklar inhämtades från litteratursökningar i två databaser. Artiklarna granskades med hjälp av Sophiahemmet högskolas bedömningsunderlag. Artiklarna sammanställdes och analyserades genom en integrerad dataanalys. 

Resultat

Resultatet sammanställdes till fyra huvudkategorier vilka benämndes psykiska, kognitiva och fysiska komplikationer samt generell hälsa. Personerna upplever bristfällig adekvat information från vården om eventuella komplikationer. Resultatet visar att personer som haft sepsis i stor utsträckning lider av komplikationer i samtliga aspekter och upplever en försämrad livskvalitet i efterförloppet. I resultatet framkom även att personer som haft sepsis ofta har ett stort behov av stöttning för att klara sitt vardagliga liv, från såväl närstående som från vården.

Slutsats

Studien visade att personer som haft sepsis har erfarenheter av ett antal olika typer av komplikationer. Kunskap kring komplikationer efter sepsis verkar vara bristande hos sjuksköterskor såväl som hos annan hälso- och sjukvårdspersonal. Det finns ett stort behov av information om dessa komplikationer, uppföljning samt eftervård inom denna patientgrupp. Detta trots att en betydande andel av riktlinjerna kring sepsisvård belyser behovet av uppföljning och eftervård.

Background

Sepsis is a life-threatening condition with dysfunctional impact on at least one organ system caused by a systemic abnormal reaction to an infection and is associated with high mortality. In the last years there has been an improvement in terms of guidelines for early detection and treatment. There has been a growing interest in follow-up after sepsis both in research as well as in new guidelines. However, the knowledge of complications after sepsis among nurses and other healthcare professionals is still reported to be insufficient. Dorothea Orems self-care theories was used as theoretical framework for this review.

Aim

The aim was to review complications following sepsis in adults. 

Method

We conducted a non-systematic literature review based on 14 published articles using both qualitative and quantitative approaches, which were retrieved from literature searches from two databases. The articles were reviewed using Sophiahemmet University’s assessment basis and were compiled and analyzed through an integrated data analysis. 

Results

The results were compiled into four main categories: psychological, cognitive and physical complications and general health. The findings indicate that individuals often suffer from complications across all these domains and experience a reduced quality of life following sepsis. Furthermore, the results show that these individuals experience large need of support in their daily life following sepsis, both from their families and healthcare providers.

Conclusions

The study showed that individuals who have had sepsis have a high frequency of different types of complications. There seems to exist a gap of knowledge about complications after sepsis among nurses and healthcare personnel. The need to strengthen the education about these complications and follow-up among this group of patients seems obvious, despite the fact that a significant proportion of the guidelines for sepsis care highlight the need for follow-up and aftercare.
