Emergency care from childrens’ and parents' perspectives : a literature review (English)
Abstract [en]

Background: A child that requires acute medical attention differs from an adult in need of emergency care. This is because a child’s physiology and anatomy are significantly different. Essentially this means that treatment and the measurements that are taken to help assess the general physical health also differs. Still, children often visit adult emergency departments and are taken care of by personnel that aren’t used to taking care of children throughout Sweden, as well as throughout the world. There are only a handful of specialized pediatric emergency departments. Broad knowledge is expected of frontline emergency healthcare workers. More information is needed of children's and parents' experiences within emergency care to better understand if the care given is sufficient and if there are factors that need to be taken into consideration in order to improve care within the emergency services sector.

The aim of this study was to explore different factors that ultimately can affect acutely ill children’s and their parents’ experiences of emergency care and to then highlight these factors by applying Callista Roy's nursing theory.

Method: A literature review with an inductive approach. 17 articles were included in the study in which were both qualitative and quantitative articles. Integrated analysis was then used.

Results: Eight categories emerged from the results which were then distributed in Callista Roy’s four main modes in the adaptation model of nursing. The eight categories that were formed are as follows: Internal and external environment, child-centered care, family-centered care, safety/security, involvement, shared information, and collaboration.

Conclusion: Children's experiences are lacking in research due to ethical concerns. The child perspective is revealed to some degree in this study, but many times through parents’ own words. What is conveyed through the results is that there are many aspects to consider in order to deliver sufficient nursing care. It is important to give family-centered care in which all family members are supported and that the child is heard and involved to the fullest extent possible.

Bakgrund: Ett barns behov av akutsjukvård skiljer sig från vuxnas relaterat till att barnets fysiologi och anatomi väsentligen skiljer sig från vuxnas anatomi och fysiologi. Vilket i sin tur påverkar bedömning, vitala parametrar samt behandling av barnet. Bred kompetens förväntas av sjuksköterskan prehospitalt samt på akutmottagning dessutom har många sjuksköterskor mer erfarenhet och kunskap av vuxna patienter än av barn. Detta berör Sverige och flera andra delar av världen. Det finns bara ett fåtal specialiserade barnakutmottagningar. Det behövs en fördjupad förståelse gällande barns och deras föräldrars erfarenheter och upplevelse inom akutvården för att identifiera faktorer som behöver beaktas för en förbättrad omvårdnad inom akutsjukvården med fokus prehospitalt samt på akutmottagning.

Syftet var att beskriva faktorer som påverkar akut sjuka barns och deras föräldrars upplevelse av akutsjukvård belyst utifrån Callista Roys omvårdnadsteori.

Metod: En litteraturöversikt med induktiv ansats där 17 vetenskapliga kvalitativa och kvantitativa artiklar inkluderades i resultatet. Integrerad analys användes.

Resultat: Åtta kategorier framkom ur resultaten som sedan fördelades i Callista Roys fyra domäner för omvårdnad. De åtta kategorierna som bildades var följande: Inre och yttre miljö, barncentrerad vård (child centered care), familjecentrerad vård, säkerhet, delaktighet, delad information och samarbete.

Slutsats: Barns erfarenheter saknas i forskningen på grund av etiska betänkligheter. Barnets perspektiv avslöjas till viss del i denna studie, men många gånger genom föräldrarnas egna ord. Det som förmedlas genom resultaten är att det finns många aspekter att ta hänsyn till för att kunna leverera tillräcklig omvårdnad. Det är viktigt att ge familjecentrerad vård där alla familjemedlemmar stöds och att barnet hörs och involveras i största möjliga utsträckning.
