Bakgrund: Inom palliativ vård hålls svåra och existentiella samtal. Att hålla och bemöta dessa samtal kräver yrkeskunskap och erfarenhet. Beroende på olika faktorer så har sjuksköterskorna olika förutsättningar för dessa samtal. I arbetet har Jean Watson teori har använts som teoretisk utgångspunkt.

Syfte: Att belysa faktorer som påverkar sjuksköterskans upplevelse av att hålla svåra samtal i livets slutskede.

Metod: En systematisk kvalitativ litteraturöversikt med induktiv ansats. Databaserna PubMed och CHINAL har använts. 15 artiklar valdes ut som svarade på syftet.

Resultat:I resultatet av litteraturöversikt framkom sjuksköterskans upplevelse av bristande kunskap och avsaknad av utbildning i samband med svåra samtal. Avsaknad av utbildning kan resultera i osäkerhet och låg självkänsla. Arbetserfarenhet och trygghet i yrkesrollen kan bidra till att sjuksköterskan upplever sig bekväm med att hålla svåra samtal. Svåra samtal kan väcka etiska tankar, vilka behöver diskuteras i arbetsgruppen. Om inte etiska tankar uppmärksammas kan det uppstå meningsskiljaktigheter om patientens vård. Olika strategier kan användas av sjuksköterskan då hen upplever sig osäker på i vilken omfattning av information patienten fått eller hur mycket av informationen som uppfattats av patienten.  Slutsats:Kunskap och utbildning behövs för att hålla svåra samtal, det kan påverka upplevelsen av samtalet. Etiska dilemman behöver diskuteras då det kan stärka sjuksköterskans förhållningssätt. Ämnet är snävt och fler studier behövs, därav behöver ämnet belysas och diskuteras på arbetsplatser för att lyfta ämnets betydelse. 

Abstract 

Background: Within palliative care difficult and existential conversations often take place. To conduct and meet these conversations, knowledge of the occupation and work life experience is required. Depending on a variation of factors the nurses conduct these conversations under different circumstances. Jean Warson has been used as a theoretical starting point.

Aim:To acknowledge the factors that affect the nurse’s experiences of conducting difficult conversations at life’s end.

Method: A systematic overview of qualitative literature with an inductive approach.The databases used were PubMed and CHINAL. 15 articles were thenchosen that answered the aim of the study.

Results: The result of the literature study shows that lack of knowledge and missing education affects the nurse’s experience in conjunction of difficult conversations. The lack of education can result in insecurities and low selfesteem. Work life experience and a sense of security in the professional role can contribute to the nurse’s overall comfort in partaking in difficult conversations. Difficult conversations can bring up ethical thoughts, which needs to be properly discussed withing a work group. If the ethical thoughts aren’t properly acknowledged disputes about the patient’s care may arise. Different strategies can be applied by the nurse if they are unsure of the extent of the information the patient has received or how much has been understood.
